1. You do not have to use our fill-out sheet. Use anything you like, as long as you get the basics. Name, link, description, etc.
2. You must have out banner up first; no banner, no affiliate.
3. Please put out banner in a place in which it can be seen. It kind of defeats the purpose if no one knows where it is.
4. Just being able to copy and paste the code is very, very nice. Please include a full code.
5. I don't care if your site is the greatest in the world; if a guest can't affiliate on it, I'm not going to take the time to put your site up.
6. Please have your site banner with a standard 88x31 size, please. The affiliate box looks prettier that way.
